2023 – 2024 Wellness Incentive Program & Insurance Premium Discount
At Atlantic, we care about your health and well-being. We want to continue to empower all of you to take strides towards living a healthier life. One of the main areas that is important to focus on is preventive care. Think about preventive care like taking care of your car. You don’t know what is going on until you look under the hood and each year you need to change the oil, rotate the tires, and fill it with gas in order for it to run well and last a long time. Your body requires the same amount of care.
We believe that the best healthcare is preventive healthcare. It is important that everyone in the Atlantic family have a healthcare provider who can establish a baseline for your health and be there to help identify early signs of cancer, chronic disease and even depression or anxiety. Therefore we will continue our Wellness Incentive Program that allows you to earn a discount on your medical premium by completing these 4 preventive activities: Biometric Screening, Annual Physical, Preventive Exam, and a Health Coaching or Behavioral Health Visit.
How to Earn The Wellness Discount
Complete these four activities before the August 31, 2024 deadline to earn the wellness discount for next year.
Forms are required for confirmation of the Annual Physical and the Preventive Exam.

1) Complete a Biometric Screening
- Onsite biometric screenings will be held at most branches in October and November 2023.
- Please take the 2024 Annual Physical and Biometric Screening Form with you to the doctor. Once complete, this form can be sent directly to Marathon Health at wellness@marathon-health.com or by fax at 802-419-9688.
** If you complete your biometric screening at your doctor, the activity will not be marked complete until the Annual Physical and Biometric Screening form is received by Marathon Health.
2) Complete an Annual Physical at the Health Center (Tabor City Employees) or with your own Primary Care Doctor.
- Please take the 2024 Annual Physical and Biometric Screening Form with you to the doctor. Once complete, this form can be sent directly to Marathon Health at wellness@marathon-health.com or by fax at 802-419-9688.
** This activity will not be marked complete until the Annual Physical and Biometric Screening form is received by Marathon Health. - Need help finding a primary care provider who accepts Atlantic insurance? 3) Complete One Preventive Age/Gender Indicated Health Exam, Dental Cleaning, or Eye Exam
- Please take the 2024 Preventive Screening Provider Verification Form with you to your Preventive Exam. Once complete, this form can be sent directly to Marathon Health at wellness@marathon-health.com or by fax at 802-419-9688.
**The activity will not be marked complete until the Preventive Screening Provider form is received by Marathon Health.These following activities qualify as a preventive exam:
- Mammograms
- Colonoscopies
- Well-woman
- Prostate Exam
- Dental Exam/ Screening
- Prev Skin Check
- Vision (Eye) Exam
4) Complete One Health Coaching Visit or Behavioral Health Appointment
- You can make an appointment in the Marathon Health portal with your health coach or Tiffany. You can also call them directly. For more information about Health Coaching click HERE. For more information about Tiffany, our Behavioral Health Counselor, click HERE.
If you missed the Aug. 31st deadline, that’s OK. If you are paying for the standard insurance rate, we will adjust you to the discounted rate during the current plan year, once you have completed all four activities. Marathon Health will provide a monthly incentive report and any employee who is currently at the standard rate for insurance will be moved to the wellness rate the following month. For example, if you complete the four activities and return all your forms on November 15th, we will pull a report at the beginning of December and move you to the wellness rate for the remainder of the program year.

Narus Health Concierge Service is here to help! The free concierge service works with Lucent and versed in our insurance plan. Spouse iPad Raffle
We want you to encourage your spouses to also get their annual physical! As an extra incentive, we are doing a raffle for an iPad for any spouses who are on company insurance and get their yearly physical by the August 31st deadline.
Early Bird Raffle
We will be doing a raffle drawing in May for an iPad for anyone who has completed their 4 activities by April 30th. Anyone who is eligible for this drawing will also have their names entered into the Wellness Drawing in August.
Marathon Health Portal
You can check the status of your incentive on the Marathon Health Portal. If you forgot your username or password, you can click those links on the login page to reset. Need additional help? Call Marathon Health directly at (866)434-3255.
